Il ruolo del tecnico della riabilitazione psichiatrica nell'ADHD: l'esperienza del parent training presso il servizio di neuropsichiatria infantile di Pinerolo

This study presents parent training as an interventional program aimed at parents to help them and support them in their parental task, in order to improve parent-child relationship and their communication. It also aims to highlight how the Psychiatric Rehabilitation Technician can be a central figure within these groups. The first chapter gives a detailed framework of ADHD according to the scientific literature; it will present a brief history of ADHD, the symptomatic picture of the disorder, describing how symptoms manifest in everyday life, and then propose a vision on the course and on the etiology of the disorder. Furthermore, the role of the executive function in the pathophysiology of the disorder will be highlighted, offering a view on the different models concerning the latter. In the second chapter the different diagnostic modalities will be examined, in terms of procedures and diagnostic manuals, followed by the main co-morbidities. The third chapter will address the different therapeutic approaches which will be divided in relation to the target and the therapeutic program highlighting the need for multimodal treatment for ADHD subjects. The fourth chapter will concern the interventions addressed to parents, known as Parent Training. Moreover in this case there will be a brief historical part concerning its birth and its development; it will also offer an overview of program, illustrating the organization and structuring of these groups. The fifth chapter will be dedicated to the group of Parent Training, structured according to the CERG program (Cognitive, Emotional and Relational support of Parents), held at the ASL TO3 of Pinerolo during the training internship, offering a vision on the structuring and what emerged in the various meetings in relation to the issues addressed. During the intervention, the Conner's Rating Scale and the Parenting Stress Index were delivered in the pre and post-treatment, to ascertain whether a change was induced during the various encounters. Therefore, a subparagraph will be present which will analyze and verify if there have been improvements in relation to the different items, in the two scales, following the parent training The last chapter will examine the central role of the Psychiatric Rehabilitation Technician in a Parent Training program. This chapter will highlight the presence of this professional figure in the juvenile context in the various structures on the territory of the Piemonte Region through a resolution of the Regional Council of 18 December 2012; The different evidence-based techniques will be uncovered, including family psychoeducation, a technique which can be used by the Psychiatric Rehabilitation Technician and which includes parent training.

Il lavoro di tesi intende presentare il parent training come programma di intervento rivolto ai genitori per aiutarli e sostenerli nel loro compito genitoriale, al fine di migliorare la relazione genitori-figli e la loro comunicazione. Inoltre si pone l'obiettivo di evidenziare come il Tecnico della Riabilitazione Psichiatrica possa essere una figura centrale all'interno di tali gruppi. Il primo capitolo desidera offrire un'inquadramento dell'ADHD secondo la letteratura scientifica; presenterà un breve excursus storico, il quadro sintomatologico del disturbo, descrivendo come i sintomi si manifestano nella quotidianità, per poi proporre una visione sul decorso e sull'eziologia del disturbo. Inoltre verrà messo in evidenza il ruolo delle funzioni esecutive nella fisiopatologia del disturbo, offrendo una visione sui diversi modelli riguardanti quest'ultime. Nel secondo capitolo verranno prese in rassegna le diverse modalità diagnostiche, in termini di iter e manuali diagnostici per poi considerare le principali comorbilità. Il terzo capitolo affronterà i diversi approcci terapeutici i quali saranno divisi in relazione al target e al programma terapeutico evidenziando la necessità di un trattamento multimodale per soggetti con ADHD. Il quarto capitolo riguarderà l'intervento indirizzato ai genitori, il Parent Training. Anche in questo caso vi sarà una breve parte storica relativa alla sua nascita e al suo sviluppo; inoltre offrirà una panoramica del programma CERG, illustrando organizzazione e strutturazione di tali gruppi. Il quinto capitolo sarà dedicato al gruppo di Parent Training, strutturato secondo il programma CERG (sostegno Cognitivo, Emotivo e Relazionale dei Genitori), tenutisi presso l'ASL TO3 di Pinerolo durante il tirocinio formativo, offfrendo una visione sulla strutturazione e su ciò che è emerso nei diversi incontri in relazione alle tematiche affrontate. Durante il percorso sono stati consegnati, nel pre e post trattamento, le scale Conner's Rating Scale e Parenting Stress Index, per appurare se nel corso dei vari incontri è stato indotto un cambiamento. Sarà dunque presente un sottoparagrafo che andrà ad analizzare e a verificare se sono avvenuti dei miglioramenti rispetto ai diversi items, nelle due scale, in seguito al parent training. L'ultimo capitolo prenderà in esame il ruolo centrale che potrebbe avere la figura del Tecnico della Riabilitazione Psichiatrica in un programma di Parent Training. All'interno di questo capitolo verrà evidenziata la presenza di questa figura professionale nel contesto minorile nelle diverse strutture presenti sul territorio della Regione Piemonte tramite una Delibera della Giunta Regionale del 18 dicembre 2012; Verranno esposte le diverse tecniche evidence-based, tra cui la psicoeducazione familiare, tecnica che può essere utilizzata dal Tecnico della Riabilitazione Psichiatrica e che include il parent training.
