Kangaroo babies: il primo contatto e la marsupioterapia con il neonato pretermine

The heart of this thesis is the Kangaroo Mother Care (KMC), a technique utilized to strengthen the bond between mother and child. It is an innovative modality that allows a growth in closeness between newborns (especially when they're preterm) and their caregivers. It is hoped that the skin-to skin contact between the dyad will happen since the first moments of the newborn's life, as it happens when the birth is without complications, when the family can serenely start their new life with the baby. The ties created in this moment become fundamental for the future attachment. The family who faces complications, such as premature birth, requires more attentions. With a premature baby everything becomes more complicated, and the parents will need a support structure to face and overcome this initial hurdle. For this reason more attention should be paid to the Neonatal Intensive Care Unit (NICU), and it's staffing. The premature newborns' treatment has to be targeted to their needs, so that their development won't be hindered. Even after the difficult initial period, constant attention should be paid to the babies, especially at the moment in which they start doing to a nursery, when the separation between them and their parents start. The attention paid to the Kangaroo Mother Care is due to it's effective advantages, not only to the newborns but alto to their parents. This technique doesn't require any equipment and it's therefore very easy to actualise, allowing the constant skin-to-skin contact between newborn and caregiver, which is fundamental for any emotional relationship and future attachment.

Il cuore di questa tesi è la marsupioterapia, tecnica utilizzata per sostenere il primo legame del neonato con la madre. Si tratta di una modalità innovativa che permette la vicinanza tra il neonato, soprattutto quando è pretermine, e il suo caregiver. Si auspica che il contatto corporeo della diade avvenga fin dai primi momenti di vita: sicuramente ciò accade nel caso di nascita senza complicazioni, quando la famiglia può iniziare serenamente la nuova vita insieme al figlio. Il legame che si sviluppa in questi primi momenti risulta fondamentale per il futuro attaccamento. Richiede maggior attenzione la famiglia che affronta una nascita difficile, come nel caso di un bambino che nasce prima del tempo. Con un neonato prematuro tutto appare più difficoltoso e gli stessi genitori necessitano di un sostegno costante affinché riescano a sviluppare la resilienza, indispensabile per superare questo iniziale ostacolo. Per tale motivo dovrebbe aumentare l'attenzione nei confronti dei reparti di Terapia Intensiva Neonatale (TIN) nonché del personale presente al suo interno. Le stesse cure da dedicare al piccolo devono essere più attente ai suoi bisogni, che sono differenti da quelli di un nato a termine, affinché il suo sviluppo possa procedere senza problemi. Anche dopo il difficile periodo iniziale è indispensabile continuare a rivolgere al bambino le stesse attenzioni, soprattutto nel momento in cui viene inserito al nido, proprio per il distacco che si verifica in quel momento tra lui e il genitore. L'attenzione posta sulla marsupioterapia è legata agli effettivi vantaggi che derivano da tale tecnica, che interessano sia il neonato che i suoi genitori. Il fatto di non necessitare di alcuna attrezzatura per la sua realizzazione la rende molto semplice nell'utilizzo, permettendo un costante contatto corporeo tra il neonato e il caregiver, basilare per qualsiasi relazione affettiva e di attaccamento.
