L’UTILIZZO DELL’INFERMIERE NELLE RIVALUTAZIONI NEUROLOGICHE TRAMITE LA SOMMINISTRAZIONE DEL N.I.H.S.S. (National Istitutes of Health Stroke Scale)

Introduction: the professional figure of the nurse is increasingly used in specific areas of expertise, in my case I chose to investigate the role of the nurse in the performance of re-evaluations through the use of the NIHSS (National Institutes of Health Stroke Scale) in patients with acute stroke. The decision to investigate this field stems from direct observation of the figure of the nurse in the emergency department in Italy, which can not administer this scale if not in possession of specific master, unlike what happens in other realities, such as the U.S. and Canada, where nurses are given this responsibility for years. Materials and methods: the search was carried out on the PubMed and CINAHL databases by combining through Boolean operators AND and the MeSH terms "nurses", "NIH" and "stroke". In a second step, a search was conducted on the PubMed site using the free terms "NIHSS AND Nurse". Results: the search led to the analysis of 7 scientific studies relevant to the research question. Discussion: the results obtained from the analysis of the studies examined show that the role of the professional figure of the nurse could be of help to the neurologist in the administration of the NIHSS scale for the neurological reassessment of patients with stroke, who need repeated assessments of neurological status both during therapy and in the post-treatment period. Conclusion: it can be attested that the professional figure of the nurse, with a specialized training in stroke, could be of great help to the neurologist in the re-evaluations through the administration of the NIHSS scale; however, further studies are needed, particularly at European level, to support the currently existing data.

Introduzione: la figura professionale dell'infermiere è sempre più utilizzata in ambiti di competenza specifici, nel mio caso ho scelto di approfondire il ruolo dell'infermiere nell'effettuazione delle rivalutazioni attraverso l'impiego della scala NIHSS (National Institutes of Health Stroke Scale) nei pazienti con stroke acuto. La scelta di approfondire questo campo nasce dall'osservazione diretta della figura dell'infermiere in pronto soccorso in Italia, che non può somministrare tale scala se non in possesso di master specifici, a differenza di quanto avviene in altre realtà, come quella statunitense e canadese, dove agli infermieri viene attribuita questa responsabilità già da anni. Materiali e metodi: la ricerca è stata effettuata sulle banche dati PubMed e CINAHL combinando mediante gli operatori booleani AND e i termini MeSH “nurses”, “NIH” e “stroke”. In un secondo momento è stata condotta una ricerca sul sito PubMed tramite l’utilizzo dei termini liberi “NIHSS AND Nurse”. Risultati: la ricerca ha portato all’analisi di 7 studi scientifici pertinenti al quesito di ricerca. Discussione: i risultati ottenuti dall’analisi degli studi presi in esame dimostrano che il ruolo della figura professionale dell’infermiere potrebbe essere di aiuto al neurologo nella somministrazione della scala NIHSS per le rivalutazioni neurologiche dei pazienti con stroke, i quali necessitano di ripetute valutazioni dello stato neurologico sia in corso di terapia, sia nel periodo successivo al trattamento. Conclusione: si può attestare che la figura professionale dell’infermiere, con una formazione specializzata nello stroke, potrebbe essere di grande aiuto al medico neurologo nelle rivalutazioni tramite la somministrazione della scala NIHSS; tuttavia, sono necessari ulteriori studi, in particolare a livello europeo, a supporto dei dati attualmente esistenti.
