Quando il corpo parla: la dermatite psicosomatica come voce di un disagio

The main focus of the following thesis work is the discussion of psychosomatic disorders in childhood, and in particular psychosomatic dermatitis. The decision to develop this thesis was determined by a personal curiosity, combined with the certainty that skin diseases have a possible psychosomatic origin, particularly atopic dermatitis. Confirmation of my hypothesis was carved out through the analysis of research by various authors. The aim of the present paper is to investigate the possible relationship between dermatitis, seen as the outcome of a psychosomatic disorder, and the difficulties that can arise in the emotional and relational field in childhood. The intention is to understand how much emotional suffering can influence the clinical condition or how much it is the physical condition that creates emotional distress. In the medical field, unfortunately, there is still a disconnect between physical and mental conditions. With this work, we attempt to delve as deeply as possible into the connections that may exist, thus trying to shed light on the issue. The first chapter begins with a brief introduction to psychosomatic disorders in childhood, followed by an in-depth look at the relationship between mind and body and how, throughout history, there has been an evolution from their separation to their integration, and the relationship between emotional expression and illness; there is also an analysis of the psychosomatic personality and the concept of alexithymia. The chapter concludes with a brief introduction on the diagnosis of disorders and an analysis of the manuals that deal with them. The second chapter will be devoted to a specific disorder, namely psychosomatic dermatitis, with its forms of manifestation, a comparison of psychotherapeutic approach perspectives, keeping the focus on psychoanalytic and psychodynamic ones, and the characteristics of the child patient facing this disorder. The focus of the third chapter will be childhood atopic dermatitis, with specific reference to a clinical case of a seven-year-old child. The chapter will conclude with a commentary on the case, including treatment for the disorder.

Il focus principale del seguente lavoro di tesi è la discussione dei disturbi psicosomatici in età infantile, e in particolare della dermatite psicosomatica. La decisione di sviluppare questa tesi è stata determinata da una curiosità personale, unita alla sicurezza che le malattie della pelle abbiano una possibile origine psicosomatica, in particolare la dermatite atopica. È stata carcata una conferma della mia ipotesi attraverso l’analisi delle ricerche elaborate da diversi autori. Il presente elaborato si pone come obbiettivo quello di indagare la possibile relazione tra la dermatite, vista come esito di un disturbo psicosomatico e le difficoltà che possono insorgere in campo emotivo e relazionale in età evolutiva. L’intento è quello di capire quanto la sofferenza emotiva possa influenzare la condizione clinica oppure quanto invece sia la condizione fisica a creare disagio emotivo. Nel campo medico purtroppo esiste ancora il distacco tra condizioni fisiche e mentali, con questo lavoro cerchiamo di approfondire il più possibile i collegamenti che possono esserci, cercando così di dare luce alla problematica. Il primo capitolo iniza con una breve introduzione ai disturbi psicosomatici in età infantile, seguita da un approfondimento del rapporto tra mente e corpo e di come, nel corso della storia, vi sia stata un’evoluzione dalla loro separazione all’integrazione, fino ad arrivare alla relazione tra espressione emotiva e malattia; è anche presente una analisi della personalità psicosomatica e del concetto di alessitimia. Il capitolo si conclude con una breve introduzione sulla diagnosi dei disturbi e un’analisi dei manuali che li trattano. Il secondo capitolo sarà dedicato ad un disturbo specifico, ovvero la dermatite psicosomatica, con annesse le sue forme di manifestazione, un confronto sulle prospettive di approccio psicterapeutiche, mantendendo il focus su quelle psicoanalitiche e psicodinamiche, e sulle caratteristiche del paziente bambino che affronta questo disturbo. Il focus del terzo capitolo sarà la dermatite atopica infantile, con un riferimento specifico ad un caso clinico di un bambino di sette anni. Si concluderà il capitolo con un commento al caso, comprendente del trattamento per il disturbo.
