L'ENDOMETRIOSI: UNA MALATTIA ENIGMATICA. CONSEGUENZE PSICOLOGICHE NELLA DONNA, NELL'UOMO E NELLA COPPIA

In this dissertation we study endometriosis, a widespread pathology among the female population in fertile age: whilst the physical problems it provokes are quite well known, its psychological consequences have not been sufficiently studied yet. Starting from a general review of the national and international literature on the sickness, from the medical point of view, our study puts the emphasis on the problems that characterize the pathology: a remarkable diagnostic delay that may sometimes exceeds 10 years, an not yet well-defined etiopathogenesis, an approximative diagnostic methodology, and a therapy only aimed to the control of itself, often not resolutive. Successively, our analysis focuses on the life experience of women affected by endometriosis, in relation to the emotional repercussions that a pathology of this kind may trigger, consequently damaging the psychological well-being of the woman herself. In particular, we have highlighted the aspects concerning chronic pain, alterations of the body image, sleeping and mood disorders, and problems related to infertility. After analyzing the experiences of women, we have given voice to the experiences of those who have undergone the sickness in a meaningful, albeit indirect, way: the partners. In particular we have looked into sexuality, paying attention to the experiences of women and men, first individually and then within the couple. In the last part of this work it has been underlined the importance of a multidisciplinary taking-over of the women suffering endometriosis, through which not only the medical aspects, but also the psychological ones, are taken care of, with the scope of improving the emotional well-being and the quality of life of those women.

L’elaborato intende indagare l'endometriosi, una patologia complessa e attualmente molto diffusa tra la popolazione femminile in età fertile. Sebbene sia abbastanza conosciuta per le conseguenze fisiche che può provocare, rimane ancora poco indagata per le sue ripercussioni psicologiche sul piano individuale e di coppia. A partire da una disamina complessiva della letteratura nazionale e internazionale della malattia, si è cercato di mettere l’accento sulle problematiche che caratterizzano questa patologia da un punto di vita medico: un ritardo diagnostico importante che spesso può superare i dieci anni, una eziopatogenesi ancora indefinita, una metodologia diagnostica approssimativa ed una terapia finalizzata solo al controllo della stessa, spesso non risolutiva. Successivamente l'analisi si è concentrata sui vissuti delle donne affette da endometriosi, in relazione alle ripercussioni emotive che tale patologia può innescare, andando conseguentemente ad intaccare anche il benessere psicologico della donna stessa. In particolare sono state messe in risalto gli aspetti che riguardano il dolore cronico, l'alterazione dell'immagine corporea, i disturbi del sonno, i disturbi dell'umore e le problematiche legate all'infertilità. Dopo aver analizzato i vissuti delle donne, si è voluto dar voce ai vissuti di chi la malattia la vive in modo significativo seppur indirettamente: il partner. Quindi, si è indagato l'aspetto della sessualità facendo riferimento prima ai vissuti propri della donna e dell'uomo e poi quelli della coppia. Alla fine del lavoro viene sottolineata l'importanza di una presa in carico multidisciplinare della donna affetta da endometriosi mediante la quale venga data importanza non solo all'aspetto medico, bensì anche a quello psicologico, al fine di favorire un benessere emotivo ed una buana qualità della vita della donna stessa.
