This final dissertation presents a historical perspective and an analysis of contemporary dynamics concerning madness treatments and perceptions. It focuses on the repressive and coercive systems and mechanisms which underline psychiatric approach to madness. It presents the historical evolution of the issue: from the rise of psychiatric preeminence to the controversy about the asylum-based model in the 1960s-70s. Moreover, it shows the overcoming of the "total institution" paradigm, and the process of restoring the paradigm through new institutional settings, coherent with a "third millennium" mental health and psychopharmacology approach.

Il seguente lavoro di tesi vuole essere un'analisi storica e contemporanea sulla follia e sui quei dispositivi, vecchi e nuovi, che sottostanno alla logica repressiva e coercitiva della psichiatria. Un excursus storico a partire dagli albori del potere psichiatrico per arrivare alla messa in crisi del paradigma manicomiale negli anni Sessanta-Settanta, il superamento dell'istituzione totale, fino alla sua riedificazione, sotto nuove vesti istituzionalizzate, grazie all'egida guida della salute mentale e della psicofarmacologia di terzo millennio.
