Salute in carcere: nelle mani di chi? Intersezioni tra malattia, cura e controllo nel contesto penitenziario

This research paper places prison medicine, as a discipline and as a practice, at the centre of the analysis, attempting to reconstruct its perimeters and to question the roles and powers of health professionals in the prison context. Starting from the narratives and representations of health professionals who have worked in various prison institutions, I reflected on the mediations, interferences, overlaps and conflicts that are generated around prison medical practice, in the interaction between health workers, prison staff and patients-prisoners. The critical issues that these dynamics can generate in terms of deontology and professional ethics were also analysed. Moving from the structural to the personal frameworks of the interlocutors encountered, an attempt was made to reconstruct the intricate field of action of the actors involved: doctors, nurses and prison officers who continually negotiate their own possibilities of movement, the weight of their words and actions around the practices of care in prison. For a deeper understanding of these processes, I investigated the conceptions of health and illness expressed by the interlocutors, considering them in relation to the specificities of the prison context; I also reflected upon the dynamics of credibility and trust in the therapeutic relationship between doctor-patient in prison. The weaving of these aspects made it possible to clearly delineate the critical issues that characterise access to treatment in prison and to question the solidity of the disciplinary boundaries of medical knowledge and the role of the figures who embody and act upon it.

Il presente elaborato pone al centro della ricerca la medicina penitenziaria, come disciplina e come pratica, tentando di ricostruirne i perimetri e di interrogarsi sui ruoli e i poteri delle figure sanitarie nel contesto carcerario. Partendo dalle narrazioni e rappresentazioni di professionisti della salute che hanno lavorato in diversi istituti penitenziari, si è riflettuto sulle mediazioni, le interferenze, le sopraffazioni e i conflitti che si generano attorno alla pratica medica penitenziaria, nell’interazione tra operatori sanitari, penitenziari e pazienti-detenuti. Si sono inoltre analizzate le criticità che queste dinamiche possono generare in termini di deontologia ed etica professionale. Passando dalle cornici strutturali a quelle esperienziali degli interlocutori incontrati, si è cercato di ricostruire l’intricato campo d’azione degli attori coinvolti: medici, infermieri e agenti penitenziari che continuamente contrattano le proprie possibilità di movimento, il peso delle proprie parole e delle proprie azioni attorno alle pratiche di cura in carcere. Per una comprensione più profonda di questi processi, si sono indagate le concezioni di salute e malattia che gli interlocutori hanno espresso, considerandole in relazione alle specificità del contesto penitenziario; si è riflettuto, inoltre, sulle dinamiche di credibilità e fiducia nel rapporto terapeutico tra medico-paziente in carcere. La tessitura di questi aspetti ha permesso di delineare con chiarezza le criticità che caratterizzano l’accesso alle cure in carcere e di porre in discussione la solidità dei confini disciplinari del sapere medico e del ruolo delle figure che lo incarnano e lo agiscono.
