Assistenza al paziente terminale: le esigenze formative dell'infermiere

Introduction: end-of-life care is difficult and involves specific knowledges and great management skills to handle the patient and his family. Hospital nurses do not always have adequate training to provide quality care. Aim: investigate the training needs of hospital nurses with focus on terminally ill patients care, analyze the critical issues and assess what may be the contents, typologies and methods that they consider best in education. Material and Methods: literature review by consulting the CINAHL Complete, PubMed and PsycInfo databases, including studies on adult patients in hospitals, in english language and published in the last 10 years. Results: 13 articles have been identified. The studies were divided into two categories: studies on training requirements and on training itself, studies which report criticalities from which training needs can be deducted. Discussion: the critical issues that emerged and the direct request of nurses coincide and concern the basic principles of end-of-life care, pain management, communication skills and socio-emotional management in connecting with the patient, with his family and for the healthcare staff itself. The most useful modalities for effective training are traditional ones such as lessons, courses and internships but also non traditional ones like simulations and round tables in small groups. It is desirable that in Italy the expansion of knowledge will be mainly implemented as professional update in accordance with the ECM system of continuous training. Conclusions: the post-base nurse education regarding terminal care must be improved and strengthened. This report can be the base for the development of a survey to investigate italian nurses educational needs to develop targeted educational programs. Introduzione: l’assistenza al paziente terminale è complessa e comporta competenze specifiche e grandi capacità di gestione della persona assistita e della sua famiglia. Non sempre gli infermieri ospedalieri hanno una formazione adeguata per poter fornire cure di qualità. Obiettivi: indagare i bisogni formativi degli infermieri ospedalieri riguardo all’assistenza ai pazienti nel fine vita, analizzarne le criticità e valutare quali possono essere i contenuti, la tipologia e le modalità che essi ritengono migliori per l’insegnamento. Materiali e metodi: revisione della letteratura consultando le banche dati CINAHL Complete, PubMed e PsycInfo, includendo gli studi con pazienti adulti in contesto ospedaliero, in lingua inglese e pubblicati negli ultimi 10 anni. Risultati: sono stati individuati 13 articoli. Gli studi sono stati suddivisi in due categorie: studi centrati sulle richieste formative e sulla formazione stessa, studi che riferiscono le criticità da cui possono essere dedotte le esigenze formative. Discussione: le criticità emerse e le richieste dirette degli infermieri coincidono e riguardano i principi di base delle cure di fine vita, la gestione del dolore, le capacità comunicative e di gestione socio-emotiva in relazione al paziente, alla sua famiglia e per gli operatori sanitari stessi. Le modalità utili per una formazione efficace risultano quelle tradizionali come lezioni, corsi e tirocini ma anche non tradizionali come simulazioni e discussione in piccoli gruppi. È auspicabile che in Italia l’ampliamento delle conoscenze sia attuato principalmente come aggiornamento professionale in conformità con il sistema ECM di formazione continua. Conclusioni: la formazione infermieristica post-base riguardante l’assistenza nel fine vita dovrebbe essere migliorata e fortificata. Questo elaborato può essere la base per lo sviluppo di un’indagine sulle necessità formative degli infermieri italiani per sviluppare programmi educativi mirati.
